"Epidemiologic Signatures in Cancer"

Rising cancer incidence may not reflect changes in the true occurrence of cancer but instead changes in diagnostic practice. And changes in diagnostic practice can exert a powerful influence on cancer survival statistics. Drawing on trends in a variety of cancers, Welch will explore the differences between an epidemic of a disease versus an epidemic of a diagnosis — and why those differences can make for misleading stories reported to the public.

Learning Objectives:
  1. Differentiate an epidemic of disease from an epidemic of diagnosis.
  2. Explain why rising survival statistics (e.g. 5-year survival) may not be associated with decreased mortality – but instead reflect rising incidence.
  3. Critique misleading health messaging in the media (“fake health news”)

About the presenter:

Dr. Welch is a general internist who has worked for the US Indian Health Service, the Department of Veterans Affairs, and Dartmouth. Currently, he is a Senior Researcher at the Center for Surgery and Public Health at Brigham & Women’s Hospital in Boston. For over three decades, he has been asking hard questions about his profession. His arguments are frequently counter-intuitive, even heretical, yet have regularly appeared in the country's most prestigious medical journals — Annals of Internal Medicine, Journal of the American Medical Association, the New England Journal of Medicine and the Journal of the National Cancer Institute — as well as in op-eds in the Los Angeles Times and the New York Times. His most recent book is LESS MEDICINE, MORE HEALTH – 7 Assumptions that Drive Too Much Medical Care.
Dr. Welch questions the assumption that more medical care is always better. His research has focused on the assumption as it relates to diagnosis: that the best strategy to keep people healthy is early diagnosis – and the earlier the better. He has delineated the side effects of this strategy: physicians test too often, treat too aggressively and tell too many people that they are sick. Much of his work has focused on overdiagnosis in cancer screening: in particular, screening for melanoma, thyroid, lung, breast and prostate cancer.

“Clinical use of the Piezotome in Endodontic Surgery"

Endodontists have used ultrasonics both orthograde and surgically for decades but there has been reluctance amongst endodontists to incorporate the next iteration, the Piezotome, into our routine surgical armamentarium.  The piezotome has several advantages over “traditional” ablation osteotomy including less bone damage, better vision, better hemostasis and even recycling the bone as an autologous graft, which in turn offers faster bone healing and improved healed contours.  This presentation will demonstrate the use of a piezotome in endodontic surgery and show patient outcomes over several years of clinical experience.

Topics:
This presentation will demonstrate the use of a piezotome in endodontic surgery and show patient outcomes over several years of clinical experience.

Learning Objectives:
  1. describe the key advantages of using the piezotome in endodontic surgery
  2. identify cases which will be appropriate for piezotome use in endodontic surgery
  3. learn techniques of trap door osteotomy, storage and replantation of an autologous graft as well as instruments, materials and media for use with the piezotome

“The value of a tooth for alveolar ridge management"

It is recognized that tooth retention strategies can often result in better aesthetic outcomes when compared to dental implant strategies alone. From this perspective, the value of a tooth to maintain the supporting bone and gingival tissues will be examined. The presentation will focus on root retention strategies for traumatized or non-restorable incisors in both our adult and growing patients.

Topics:  Ridge preservation

Learning Objectives:

  1. The value of a root for ridge management.
  2. How these strategies can enhance or delay dental implant placement.

“Tooth autotransplantation can help solve dilemmas in our growing patients"

We often encounter treatment planning dilemmas for growing patients presenting with complex impacted, congenitally missing, and traumatized teeth.  Traditional methods of tooth replacement are usually not applicable at this young age. Treatment options usually include extraction, space closure or space appropriation for a future dental implant.  Tooth autotransplantation can provide additional options for these patients aimed at tooth preservation and ridge management.

Topics: Tooth Autotransplantation

Learning Objectives:

  1. Surgical considerations for an optimal outcome.
  2. Orthodontic movement of the transplanted tooth.
  3. Restoration of a transplanted premolar to mimic a central incisor.
